Successful practices in strengthening patient safety and achieving high compliance in the assessment of patient safety practices at Hospital Regional de Sinop “Jorge de Abreu”

Abstract

Introduction: Patient safety is an essential dimension of healthcare quality and requires ongoing institutional strategies to prevent avoidable harm, strengthen safety culture, and improve care processes. Objective: To describe successful practices implemented by the Patient Safety Center of Hospital Regional de Sinop “Jorge de Abreu” and analyze their impact on strengthening patient safety, reducing healthcare-related incidents, and achieving high compliance in the assessment of patient safety practices. Method: This is a descriptive, retrospective study with a quantitative and qualitative approach, characterized as a successful experience report, conducted in a public medium- and high-complexity hospital in the state of Mato Grosso, Brazil. Institutional patient safety indicators, incident reports, protocol monitoring, educational actions, and active surveillance strategies implemented between 2023 and 2026 were analyzed. Results: After an institutional assessment conducted in 2024, based on 2023 data, weaknesses related to adherence to patient safety protocols were identified. Based on the situational diagnosis, actions such as document restructuring, daily bedside active surveillance, strengthening of technical visits, invasive device monitoring, sectoral bulletins, permanent education, and monthly leadership meetings were implemented. In March 2024, 901 incidents were reported; in March 2025, 808; and in March 2026, 232, representing an approximate reduction of 74.3%. The incidence of pressure injuries also decreased from 36.36% to 11%, and high compliance was achieved in the assessment of patient safety practices. Conclusion: The successful practices implemented had a positive impact on consolidating safety culture, progressively reducing healthcare-related incidents, and improving institutional compliance. This experience highlights active surveillance, permanent education, and multiprofessional integration as replicable strategies to improve healthcare quality in public hospitals.
