Abstract

Traumatic diaphragmatic hernia (TDH) is a condition characterized by the passage of abdominal structures into the thoracic cavity through an opening in the diaphragm caused by trauma. Case Description: A 49-year-old male patient was admitted to the emergency department due to spontaneous pneumothorax. He complained of abdominal pain and respiratory discomfort. Chest and abdominal imaging showed diaphragmatic hernia in the left hemothorax, with intestinal loops and discontinuity of the left diaphragmatic dome. Discussion: The treatment of HDT remains essentially surgical, and frenorrhaphy should be performed with “X” stitches25. Surgical treatment was therefore carried out using a festooned chule and 0 polypropylene thread with reinforcing “X” stitches. The patient evolved post-operatively in a severe, septic state, using vasoactive drugs, mechanical ventilation and broad-spectrum antibiotics. Under intensive care, he showed clinical improvement and was discharged from hospital after one month
