Apostas on-line e saúde mental em adultos: Revisão crítica da literatura com metanálise exploratória de ansiedade e estresse

Resumo

A expansão das plataformas de apostas exige distinguir participação recreativa, gravidade do jogo e sofrimento psicológico. Este artigo analisa a relação entre apostas on-line e saúde mental, com atenção às apostas esportivas, à depressão e ao risco suicida. Realizou-se revisão crítica com busca dirigida no PubMed, na BVS e no Portal de Periódicos CAPES, complementada por textos de periódicos e repositórios. A discussão reuniu 18 estudos primários e sete sínteses científicas. A síntese quantitativa exploratória reuniu dois estudos transversais em adultos, com 6.283 participantes, que informaram correlações de Spearman entre gravidade do jogo on-line e ansiedade ou estresse. Utilizaram-se transformação de Fisher, efeitos aleatórios de DerSimonian–Laird e intervalos conservadores de Hartung–Knapp modificados. As estimativas combinadas foram 0,117 para ansiedade e 0,091 para estresse; os intervalos conservadores incluíram ausência de associação. A heterogeneidade da ansiedade foi elevada, enquanto o resultado de estresse foi dominado pelo estudo maior. Depressão, ideação suicida e comparações entre modalidades foram examinadas narrativamente, por incompatibilidade das medidas. A síntese narrativa sustenta uma crítica à promoção irrestrita das apostas, com efeitos experimentais de incentivos sobre decisões e evidência de danos financeiros. A metanálise não estabelece causalidade ou risco específico de plataformas. Conclui-se pela necessidade de estudos longitudinais brasileiros, medidas por produto e avaliações clínicas que considerem perdas financeiras, controle do comportamento e comorbidades.
