Fibrilação atrial subclínica detectada por dispositivos implantáveis e wearables: revisão sistemática sobre definições diagnósticas e quando anticoagular

A fibrilação atrial subclínica (FA-S), frequentemente identificada como episódios de alta frequência atrial (AHRE) detectados por dispositivos cardíacos implantáveis e tecnologias vestíveis, associa-se a aumento do risco tromboembólico. Entretanto, a decisão de anticoagular permanece controversa, uma vez que o risco absoluto de eventos tromboembólicos é variável e o uso de anticoagulantes orais implica aumento de sangramento maior. Ensaios clínicos randomizados recentes demonstram redução de acidente vascular cerebral e embolia sistêmica às custas de maior risco hemorrágico, reforçando a necessidade de individualização da conduta.
