Abstract

The expansion of telehealth in Brazil establishes a concrete alternative to mitigate geographic distances that affect vulnerable and isolated populations, promoting the decentralization of medical care within the Unified Health System (SUS). Conducting this study is justified by the need to critically evaluate the advancements and bottlenecks of this care modality, providing theoretical insights to refine public policies aimed at reducing regional disparities in healthcare access. The overall objective of this study was to analyze the impact of remote care on the consolidation of Primary Health Care in isolated regions and, as specific objectives, to identify the main technological, regulatory, and training hurdles that shape the equity of this model. The methodology consisted of a qualitative narrative review conducted in the PubMed, SciELO, and Google Scholar databases, using the descriptors "Telehealth", "Telemedicine", and "Rural Areas", with a publication timeframe between 2022 and 2026. Results and discussions indicate that telehealth has reconfigured care flows by reducing wait times in the Regulation System and eliminating exhaustive transportation costs for rural users. Telediagnosis and teleconsultations improved the monitoring of chronic conditions and strengthened local resolution capacity, supported by a consolidated regulatory framework. However, severe regional asymmetries persist due to unstable connectivity, a lack of technological infrastructure, and digital literacy limitations in rural areas, where the presence of Community Health Agents ensures treatment adherence and humanized care. In conclusion, telemedicine has established itself as a structural and permanent tool for expanding medical coverage to inner regions and promoting health equity. Nonetheless, its full consolidation as a driver of healthcare justice requires continuous intersectoral public policies focused on fiber-optic investment, training of local teams, and the ongoing strengthening of Primary Health Care in vulnerable territories.
