Telemedicine has emerged as an innovative and strategic tool in the management and monitoring of chronic non-communicable diseases (NCDs) in primary health care (PHC). This article highlights the benefits, challenges and impacts of telemedicine in the care of patients with NCDs in PHC. The study was carried out by searching the Google Scholar, Medline/PubMed and JAMA databases. The research question and strategy used were based on the PICO model. Clinical trial studies, meta-analysis, randomized controlled trials, articles, books and documents published in English, Spanish and Portuguese in the last 5 years were included. A total of 2,963 articles were initially found. After analysis and elimination of duplicates, 133 references were analyzed by title and abstract, of which 73 were included for full reading. After this stage, 58 references were excluded. In the end, 15 studies were considered eligible. The available evidence shows that the integration of telemedicine into PHC represents a significant advance in the management of NCDs, making it a promising tool which, if strategically integrated into PHC, can improve the quality of life of patients with NCDs, as well as transforming health care.
