The electrocardiogram (ECG) is an essential tool for cardiovascular evaluation in elite athletes, widely used in the screening of cardiac conditions and monitoring of physical performance. However, physiological adaptations to intense training result in electrocardiographic changes that, although benign, can be mistaken for pathological conditions. This review article describes the main benign findings on the ECG of athletes, discusses their physiological basis, and explores the complementary role of echocardiography in distinguishing between normal and pathological patterns. It is aimed at clinicians and cardiologists, with the goal of improving differential diagnosis and optimizing the management of these individuals.
