Acute chronic patient: a case report from the perspective of primary health care

Abstract

Due to the Brazilian demographic transition, the prevalence of chronic non-communicable diseases has been continually increasing, which calls into question the current care arrangements of the Unified Health System. In this sense, Primary Care plays a central role, becoming capable of acting for through health promotion and prevention and, thus, mitigating the negative outcomes of chronic diseases. However, if practice at the first level of care does not occur as recommended, users will be those most harmed. Therefore, the objective of this study is to report the care provided to a user with a chronic non-communicable disease with successive acute hospitalization events from the perspective of Primary Care. Therefore, there is a need to restructure work processes throughout the Health Care Network in order to guarantee timely monitoring of the individual, in addition to promoting training through ongoing education on the practice of palliative care at the first level of attention. It is also necessary to provide person-centered care, understanding their particularities and social determinants of health for the family health team to enable individualized and oriented care for that user, ensuring a better quality of life. and lower risk of negative outcomes.
