Abstract

A 28-year-old female patient from the interior of Bahia presented with severe symptoms, reflecting an atypical version of Cushing's disease that has the crucial need for early diagnosis. In this article, we discuss the diagnostic approach to Cushing's syndrome, analyzing the information obtained from reports detailing the diagnostic and therapeutic journey. Admission was marked by intense dyspnea, confusion and chest pain, aggravated by a decade of corticosteroid use. Physical and laboratory examinations included obesity and cushingoid features, requiring intensive care. Analysis of this atypical case offers insights into the variability of the disease and the importance of adaptive diagnostic and therapeutic strategies. Initial management focused on hemodynamic stabilization and glycemic control, culminating in progressive improvements. This case reinforces the need for vigilance and flexibility in diagnosis and treatment, emphasizing the usefulness of sharing knowledge and clinical experiences to enrich medical practice and the scientific literature.
